Jacob was born with a rare heart disease called Transposition of the Great vessels. Jacob underwent 4 open heart surgeries,and 1 for heart and lung failure, during the time he was here. Jacob passed away at the age of 3 due to heart and lung failure. 

Jacob ment the world to me, he was a very special little boy. Jacob was a brave little guy and very loving and caring. He had a way of bringing a smile to anybodys face. 

Jacob also had a brother whom was 2 yrs older than him, he passed at 2 1/2 months old of heart failure.
